I am an Authorised Representative of Requesting Entity. Requesting Entity is the entity that is requesting its Company Credit Report (CCR) along with CIBIL MSME Rank (CMR) and Goods and Services Tax (GST) Financial Report (hereinafter referred to as "GST Report") from TUCL.

I, on behalf of the Requesting Entity, hereby submit an application to receive the Receiving Entity's/its Company Credit Report (CCR) along with CIBIL MSME Rank (CMR) offered by TransUnion CIBIL Limited ("TUCL"). I hereby also submit an application to receive Requesting Entity's Goods and Services Tax (GST) Financial Report (hereinafter referred to as "GST Report") to be pulled by TUCL with my consent from the Goods and Service Tax Provider (GSP)/Goods and Service Tax Portal (GST Portal). I further state that I submit the foregoing applications for receiving the said Reports through Indian MSME Helpline Pvt Ltd (hereinafter referred to as the "Company").

I, on behalf of the Requesting Entity (hereinafter referred to as "us", "our", "my", "we"), hereby confirm, agree with, and acknowledge the following:

• The Company shall be my lawfully appointed agent and he / it has agreed to be my agent for the purposes, including, without limitation, to receive the CCR along with CMR and GST Report from TUCL on our behalf and use it in the manner agreed between the Company and TUCL, and the Requesting Entity has granted its consent for the said purpose.

• We grant our unconditional consent to the Company to receive the Consumer Credit Information and Credit Products from TUCL on our behalf, and the Company has granted its consent for being appointed for the aforesaid purpose. We hereby represent and acknowledge that the Terms of Understanding in relation to the use of the CCR along with CMR, any other Credit Products, and GST Report have been agreed between us and the Company.

• We hereby expressly grant unconditional consent to, and direct, TUCL to deliver our CCR along with CMR and GST Report to the Company on our behalf.

• The GST Report is not a Product of TUCL, and we have provided express consent to TUCL to obtain our GST-related information from the GST Portal through a GSP.

• We understand that the Company cannot access the CCR along with CMR and GST Report for more than 6 months on a single Consumer Consent, and the same shall be required to be refreshed every six (6) months. The Company can store the CCR along with CMR and GST Report only for (i) a limited time period of six months or (ii) till such time the CCR along with CMR and GST Report is required to be retained to satisfy the purpose for which it was provided or (iii) until we withdraw our consent to store such CCR along with CMR and GST Report, whichever is earlier, and thereafter, the stored CCR along with CMR and GST Report shall be deleted, purged, expunged by the Company.

• We shall not hold TUCL responsible or liable for any loss, claim, liability, or damage of any kind or nature resulting from, arising out of, or related in any way to the following: a) the delivery of our CCR along with CMR and GST Report to the Company; b) any use, modification, or disclosure by the Company of the contents, in whole or in part, of our CCR along with CMR and GST Report, whether authorized or not; c) any breach of confidentiality or privacy in relation to the delivery of our CCR along with CMR and GST Report to the Company; d) for any use made by the Company which is contrary to what has been agreed between the Company and TUCL.

• We accept and acknowledge that TUCL has not made any promises or representations to us in order to induce us to provide our CCR along with CMR and GST Report or seek any consent or authorization in this regard.

• We agree that we may be required to record our consent / provide instructions electronically, and in all such cases, we understand that by clicking on the "I/We Accept" button below, we are providing "written consent" to TUCL and authorizing Company to obtain our CCR along with CMR and GST Report from our credit profile available with TUCL. Further in all such cases "By checking this box and clicking on the 'Authorize button, we agree to the terms and conditions, acknowledge receipt of TUCL privacy policy and agree to its terms, and confirm that we are authorizing the Company to obtain our CCR along with CMR and GST Report.

• We understand that in order to deliver the product to us, we hereby authorize the Company, to obtain our CCR along with CMR, Credit Products and GST Report from TUCL.

• We are aware that in case we wish to dispute any information on the TUCL CCR along with CMR, we can contact TransUnion CIBIL directly by going to https://www.cibil.com/resolve-report-inaccuracies or writing to TransUnion CIBIL at TransUnion CIBIL Limited, One World Centre, Tower 2A, 19th Floor, Senapati Bapat Marg, Elphinstone, Mumbai - 400 013 and in the event there is any dispute pertaining to the GST Report, we will not claim from or hold liable TUCL in any manner whatsoever.

• By submitting this application form and registering ourselves, we understand that we are providing express written consent and instructions to TUCL that the Company shall have the right to request and receive our information including but not limited to a copy of our CCR along with CMR and GST Report from TUCL, and from reporting agencies, at any time for so long as we have an active account with the Company. We further authorize the Company to retain a copy of our information for use in accordance with Company's Terms of Use and Privacy Policy.

• WE UNDERSTAND THAT THE PRODUCT IS PROVIDED ON AN "AS-IS", "AS AVAILABLE" BASIS AND TUCL EXPRESSLY DISCLAIMS ALL WARRANTIES, INCLUDING THE WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, AND NON-INFRINGEMENT.

• We shall not sue or otherwise make or present any demand or claim, and we irrevocably, unconditionally and entirely release, waive and forever discharge TUCL, its officers, directors, employees, agents, licensees, affiliates, successors and assigns, jointly and individually (hereinafter "Releasee"), from any and all manner of liabilities, claims, demands, losses, claims, suits, costs and expenses (including court costs and reasonable attorney fees) ("Losses"), whatsoever, in law or equity, whether known or unknown, which we ever had, now have, or in the future may have against the Releasee with respect to the submission of our CCR along with CMR and GST Report and / or our decision to provide TUCL with the authority to deliver our CCR along with CMR and GST Report to the Company. We agree to defend, indemnify, and hold harmless the Releasee from and against any and all Losses resulting from claims made against TUCL by third parties arising from and in connection with this letter.

• We agree that the terms of this confirmation letter shall be governed by the laws of India and shall be subject to the exclusive jurisdiction of the courts located in Mumbai in regard to any dispute arising hereof. TUCL is entitled to assign its rights hereunder to any third person without taking my prior written consent.

• We confirm that the executants of this letter have the necessary power and authority to execute the same. The Requesting Entity shall be bound by the representations and confirmations provided herein and the same shall not be declared invalid/ void and / or rescinded by the Requesting Entity at any time in the future. We are aware that TUCL shall rely on this representation without being required to undertake any independent verification of these powers.
